The first external pacemaker was developed in the late 1920s and early 1930s by researchers including Albert Hyman, who coined the term "artificial pacemaker." By the late 1950s, wearable external units became clinically available for temporary use. The first successful implantable pacemaker followed in 1958, with surgeon Ake Senning implanting a device designed by Rune Elmqvist in Arne Larsson in Sweden. Larsson survived decades with multiple implants, and the device is now displayed at the Karolinska Institute. Early implantable units used transistor circuits and mercury batteries, with pulse rates typically fixed between 70 and 80 beats per minute. In 2021, Medtronic reported that the Micra AV, the first leadless pacemaker with atrial sensing, received FDA approval, expanding indications beyond single-chamber ventricular pacing Medtronic. These advances reduced complications such as lead fracture, infection, and pneumothorax compared with traditional transvenous systems.

Modern Market Leaders and Global Pacemaker Industry

The global pacemaker market is dominated by a small number of multinational corporations. Medtronic, Abbott (through its St. Jude Medical division), and Boston Scientific together account for the majority of implantable pacemaker and implantable cardioverter-defibrillator procedures worldwide. In the United States, the Centers for Medicare and Medicaid Services tracks volume and reimbursement data for pacemaker implants, with more than 200,000 devices implanted annually in recent years. Regulatory approvals from the U.S. Food and Drug Administration and the European Medicines Agency shape device availability, and companies regularly publish clinical trial results supporting safety and efficacy. Boston Scientific's pacemaker portfolio includes the Accolade and Epyra pacemaker systems, while Abbott's St. Jude Medical division offers the Assurity and Accent MRI pacemakers with MRI-conditional labeling. Medtronic's Azure and Visia QT pacemakers include remote monitoring capabilities that transmit device data to clinicians through secure networks, enabling earlier detection of issues such as lead insulation failure or battery depletion
